JACT Sports Complex Forms

JACT Sports Complex requires the certain forms be completed prior to using our facility. Teams must have each individual who will be using the facility complete our Release Form. You may download and complete these forms from the links below to save time, or they will be made available to you upon arrival. Individuals and teams will not be allowed to use the facility until these forms are complete. JACT Sports Official Rules and Regulations

Please read this form prior to participating in any sports activity at JACT Sports Complex.

 

JACT Sports Release Form

This form is required for all persons participating in practice or league sports play or individual sports activities such as batting/pitching, sports practice, etc. at JACT Sports Complex.

 

JACT Sports Team Registration

If you are a coach, team captain or other team organizer, please complete this form to register your team for league play at JACT Sports Complex. Leauge play will also require each individual on your team to colmplete the JACT Sports Release Form above.

 

JACT Sports Team Waiver
Complete this form when requesting team/league rentals.

 

Inflatable Fun Zone Release

This form must be completed by the parent or guardian of minor children participating in activities in our Inflatable Fun Zone.

 

Youth Soccer Winter Skills Sessions

Our winter youth soccer skills classes will be held at JACT Sports Complex from January 21, 2012 – March 10, 2012. Please complete this form prior to attending our winter skills sessions.
